Greetings!

This has reference to the change of office address of Pinkwork Company. Pinkwork is a
registered taxpayer with registered office address at 225 Felmans Place General
Kalentong St., Brgy. Daang Bakal Mandaluyong City. Subsequently, an application for
change of address was filed by the company by reason of her transfer of office address.

At present, Pinkwork is maintaining its address at Unit A, 7 th floor, Shaw Boulevard cor.
L. Gonzales St., Brgy. Hagdan Bato Libis Mandaluyong City.That the office address at
Kalentong, having been registered with your office as the office address of Pinkwork
Company, was indicated in its official receipts.

In this regard, Pinkwork would like to respectfully request that she be allowed by this
good office to continue to use and utilize her remaining official receipts. The following
are the unused official receipts of Pinkwork Company: Booklet No. 7, Serial No. 0335-
0350 and Booklet No. 9-20 Serial No. 0401-1000.
