and existing under and by virtue of the Philippine Laws (SEC Registration

With principal office at __________________________, do hereby certify, under oath, that the
accompanying storage disk herewith submitted to the SEC has the basic and material data in the Audited
Financial Statements likewise submitted herewith in four (4) copies and duly stamped receive by the
Bureau of Internal Revenue.

This certification is issued in compliance with the requirements of the SEC under SEC Memorandum NO.
2 dated March 12, 2001.
