AFFIDAVIT
(Re: CEASATION OF EMPLOYMENT)

I, VICTORIA C. MENDOZA, of legal age, married, Filipino Citizen, and


with residential address at No. 13 Senator Street, Sta. Monica Subdivision,
Novaliches, Quezon City, after having been sworn to in accordance with law
hereby depose and say;

1. That I am a previous employee of Rainbow Dyeing Services and


Slimtex Industries Inc. from 1993 up to September 2006 due to my
resignation;

2. That since my resignation from the above employer up to the


present, I have not found any employment and/or source of
livelihood;

3. That is for this reason that I have ceased remitting my PAG-IBIG


monthly premiums;

4. That I am executing this affidavit in order to request the PAG-IBIG


Fund for offsetting of my previous loan with the said agency with
my earned contributions;

FURTHER, AFFIANT SAYETH NAUGHT.

Done this 16th day of October 2006 at Caloocan City, Philippines.
