Exception scenarios:
Scenario I
Where investor is unable to give folio registered (old) bank's attestation, then following documents needs to be
submitted along with Signature updation request:
• PAN card copy (self-attested)
• KYC to be provided
• New bank's attestation or Affidavit on Rs.100/- stamp paper requesting for new signature with notary
attestation. Notary attestation will be in original and will include notary registration number with full
address, seal and signature of the notary.
• Document for registered bank mandate stating the investor’s bank a/c number , name of investor, a/c
type, bank branch, IFSC : Cancelled original cheque /self-attested copy of bank account statement or
bank passbook (not older than 3 months)/ bank letter on the letterhead of the bank duly signed by the
branch manager/authorized personnel (not older than 3 months).
Scenario II
Where investor is unable to give folio registered (old) bank's attestation and PAN is also not updated in the folio,
then following documents needs to be submitted along with Signature updation request:
• PAN updation request letter (along with true copy of PAN card, to be verified against original during
acceptance)
• KYC verified investor, if not verified then KYC form along with proofs to be submitted (copies with original
for verification) Where the KYC address is not matching with the folio registered address, and then
documents reflecting name and address as registered in the folio (copies with original for verification).
• New bank's attestation or Affidavit on Rs.100/- stamp paper requesting for new signature with notary
attestation. Notary attestation will be in original and will include notary registration number with full
address, seal and signature of the notary.
• Document for registered bank mandate stating the investor’s bank a/c number , name of investor, a/c
type, bank branch, IFSC : Cancelled original cheque /self-attested copy of bank account statement or
bank passbook (not older than 3 months)/ bank letter on the letterhead of the bank duly signed by the
branch manager/authorized personnel (not older than 3 months).
Kindly Note :
• Signature updation request has to be given as a standalone request only and not with any other processing
request.
• In the absence of old bank a/c proof, the unit-holder shall visit the nearest AMC/CAMS branch office for IPV(In
person verification) with the documents and identity proof.
• Bank attestation will be in original and will include name of Signatory, employee code, Designation (Branch
Manager & above), Bank Full seal and Signature.
