Dear Mr. / Ms. ____________:

We are pleased to offer you the position of ____________ under Alcalde Brewery Corporation
subject to the terms and conditions provided below. You will be receiving a basic pay of
___________________________ (P_______).

1. You will undergo a six-month probationary period of employment. Before the expiration
of your probationary employment, your performance shall be evaluated by
management. Should your performance meet the reasonable standards set by
management, your employment shall be regularized.

2. Your official operating hours shall be as follows:

Monday to Saturday 3:00 pm- 12:00 am

3. As _____, you will be entitled to the following benefits:


a. SSS
b. Phil-health
c. Pag-ibig

4. Upon regularization, you will be entitled to twelve (12) days vacation leave and twelve
(12) days sick leave per year of service. Said leaves are cash convertible and paid every
salary cut-off.
Please note that this job offer and probationary employment is conditioned on the satisfactory
confirmation of all information you provided in the Employment Application and Personnel
Record and interview, which may be completed after the date of this offer.

The company reserves its right to withdraw or cancel this offer and consider your employment
terminated immediately in the event of misinterpretation or omission of information or fact.
