Dear Parent/Guardian of Krithika Vishnu

Please read all of this letter.


Important — If Krithika cannot attend this appointment or wish to cancel it, please let us
know as soon as possible either on telephone 01582 561385 or via the link at
www.ldh.nhs.uk we can then offer another patient this appointment. If you do not attend
the appointment Krithika may be discharged back to the GP, or re-join the waiting list, and
await the next available appointment. If you have any concerns about Krithika condition

A clinic appointment has been made for Krithika.
On: Monday 26th December 2022
10:00 am
Location: Zone D - Childrens Outpatients
Krithika will be seen by Dr J A Kapadia - Neonatal Intensive Care Consultant, or another
member of the Neonatal Intensive Care team. Please arrive 10 minutes early for the
appointment as Krithika may need a height and weight measurement prior to the
appointment time. Unfortunately, there is no guarantee that Krithika will be seen exactly at
the time stated or by the Consultant named.

Could you please bring along to the appointment a list of all medication
Krithika is currently on or the bottles/boxes of the medication, which have the
information of doses and frequency on it and the child health record (red
book), if appropriate.
If Krithika has recently been unwell please ring children's outpatients on (01582) 497224 to
discuss whether the appointment needs to be postponed. This is because your child may be
contagious and may put other children at risk.
