Personnel Discipline

Policy:

1. At all times, disciplinary measure imposed on erring personnel should be implemented


only after an extensive investigation and much due process.

2. The department head may impose a warning (not a disciplinary measure) in writing.
Similarly, the head can imposed a reprimand formally and up to 7 days of suspension
without pay.

3. The following penalties / disciplinary measures shall be imposed for the following:

 Grave Offenses – Outright separation and dismissal from SGH for 3rd offense, 6
months suspension for 1st offense.

 Less Grave Offenses – 1 to 2 months suspension but not less than 15 days.

 Light Offenses – From 1 to 15 days suspension depending on degree of offense.

The following shall be grounds for disciplinary action:

1. Grave Offenses

 Gross neglect of duty


 Grave misconduct
 Dishonesty
 Falsification of Records (Medical, Personal Data Sheet)
 Disloyalty or conduct prejudicial to SGH interest
 Refusal to perform official duties
 Inefficiency and incompetence in the performance of official duties
 Gross insubordination
 Oppression
 Pilferage of SGH supplies.

2. Less Grave Offences

 Simple neglect of duty


 Simple misconduct

 Drinking alcohol during office hours


 Gross discourtesy to patients/family/visitors and superiors as well as
co-employees
 Coercing a co-employee to do a misdeed involving policies and
regulations
 Soliciting extra payment for services outside those established and
paid for.

3. Light Offences

 Violation of reasonable office rules and regulations


 Refusal to render overtime work and report to duty when called for in
an emergency
 Habitual absences
 Habitual tardiness
 Gambling during office hours
 Failure to act expeditiously on requests for services
 Failure to process and complete documents within the implementing
period set by policies and standards
 Being with company of co-employees violation of office rules and
regulations, e.g. drinking or gambling during office hours.

Maintenance Monitoring of the Building

Policy:

 There must be an annual inspection conducted by the Municipal Engineering Staff that
the building was fit for occupancy and the

 Maintaining the building to preserve its architectural well being, structural stability,
electrical and mechanical safety, sanitary and fire protection facilities.

 The building shall not be used for purpose other than its approved use.

 No alteration / addition / repair / new electrical, mechanical installations shall be made


without a permit approved by DOH personnel.

Electrical

Policy:

 Routine inspection of all electrical wiring should be made twice a month by the licensed
electrician. The electrician must see to it there is no overloading of power.

 Electrician must inspect all electrical equipment and appliances and ensure of safety use.

 Electrician must inspect every room and offices to see to it that there are no octopus
connections.

 All hospital personnel must see to it that before going out of their officer must unplug the
electrical appliances used during their tour of duty especially those on 8:00am to 5:00
shift.

 Emergency light and flashlights should be used during brownouts and turn on the hospital
generator. Use of candles is strictly prohibited.

 Emergency light should be regularly inspected and plugged properly to ensure its
functions when there is no electrical power

 All employees must know where the main switch is.

Water Supply

Policy:

 The water supplies of Santos General Hospital is directly coming from Malolos Water
District – and have passed the requirements set by the Philippine National Standard
for drinking water for physical, chemical, geological, bacteriological and radiological
quality. Attached herein is the certificate of potability which was given by the quality
monitoring committee of the office of the city by health officer in the city of Malolos,
Bulacan.

 Water analysis done every year

Fire Prevention and Safety

1. Conversely, all employees must attend a seminar on fire safety, prevention and
control to be conducted by the City Municipal Fire Prevention and Control Office.
Periodic fire drills shall also be instituted.

2. The location of fire extinguishers, located strategically in the hospital, shall be


known to every employee its use and operation.

3. While smoke detectors are in optimal operating readiness in the hospital corridors,
rooms and various areas, there are time that false alarms triggered by smoke in
small amounts, as well as dusty gust of winds can set the alarm on. Every
employee must know the location of the cut-off or alarm-off knob of the alarm
system as this when left on for quite sometimes may cause panic among the
patients and staff.

4. Periodic inspection of all electrical outlets, appliances, and equipments requiring


the use of electricity for frayed wire, short circuits, shall be logged in and
corrected.

5. Unauthorized use of electrical heaters, mobile phone chargers, audio and visual
equipments, as well as microwave oven, toaster ovens, etc. is prohibited in
dormitories and employees room, private as well as wards room. The electrician in
charge that makes periodic inspections of the facilities shall take notes in a log-in
book and report to management of violations thereof. He shall recommend and
post in each facility what to allow and disallow.

6. All service area should have a list of telephone number of the Bureau of Fire and
Prevention Control, Volunteer Fire Brigades in Bulacan, as well as the Barangay
Captain and City Police for assistance and reporting purposes.

Plumbing

Policy:

 The maintenance plumber is responsible for installing, inspecting, and maintaining all
piping systems and allied n equipment in the hospital complex.

 The maintenance plumber must routinely inspect all the piping system and must be
required to work after normal working hours, weekends and holidays, work outdoors
during inclement weather conditions, day or night responds to call out when requested.

 The maintenance plumber duty check all areas of the hospital for the preventive
maintenance, repairs, replacement and installation of plumbing fixtures and equipment.

 The maintenance plumber duty check toilets, lavatories, scrub sinks, faucets and flush
valves and install. Maintain a repair if not functioning.

 Monthly checks repair and maintain the fire sprinkler system.
