Cancellation Form-Multiple Sip

This document is a request to cancel existing systematic investment plans (SIPs). It allows the investor to cancel all SIPs under their folio, or specify certain SIPs to cancel. The form includes fields to provide the investor name and folio number, as well as details of the schemes, plans, options, SIP dates and amounts for plans to be cancelled. Signatures of the investor and bank account holders are required to authorize the cancellations. An acknowledgment slip is attached to confirm receipt of the cancellation request.
