AFFIDAVIT OF CONFORMITY

I, VICTORIA J. DELA CRUZ, of legal age, married, Filipino citizen and with residence and
postal address at Block 4 Lot 178, 180 & 182, Summerfield Subd., Brgy. Osorio, Trece Martires City,
Cavite, after having been duly sworn to in accordance with law, depose and say:

1. That I am the biological mother of JOHN PAULO DELA CRUZ LONGARIA March 15, 2001
at General Emilio Aguinaldo Memorial Hospital (GEAMH), Brgy. Luciano, Trece Martires City,
Cavite;

2. That I, together with my husband and the child’s biological father, LEOPOLDO S.
LONGARIA JR., are currently processing the legitimation of our child;

3. That on June 12, 2003, my husband, LEOPOLDO S. LONGARIA JR. and I entered into
marriage at the Office of the Municipal Mayor of Town of Magalang, Province of Pampanga,
solemnized by then Municipal Mayor, Hon. Pastor Z. Guiao;

4. That I am expressing and giving my conformity for my child to use his father’s surname in
accordance to Republic Act 9255;

5. That I am executing this affidavit for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 2nd day of March, 2023 at
Trece Martires City, Cavite, Philippines.
