This document is a contract agreement between Ramon Ronald J. Veloso, the proprietor of INOPACAN FIRM FUEL SERVICE, and the Municipality of Inopacan, Province of Leyte, represented by Hon. Azucena P. CONTRACT AGREEMENT

THIS AGREEMENT IS MADE AND ENTERED BETWEEN :

RAMON RONALD J. VELOSO of legal age, married, Filipino, and a


resident of Baybay City, Leyte, owner and proprietor of INOPACAN FIRM FUEL
SERVICE hereinafter referred as The PROPRIETOR;
AND
The MUNICIPALITY OF INOPACAN, Province of Leyte, herein represented
by the Hon. AZUCENA P. MIRAMBEL, MD, FPOGS, herein referred to as The
PROCURING ENTITY;

WITNESSETH:

WHEREAS, the typhoon “ODETTE” ( internationally known as RAI )


devastated the entire municipality of Inopacan, Leyte;
WHEREAS, the entire Municipality of Inopacan, Leyte is declared under State
of Calamity;
WHEREAS, the need to purchase fuel be it gasoline or diesel is imperative for
the machines and vehicles necessary for clearing and relief operations as well as for
the generator consumption;
WHEREAS, the “PROPRIETOR is willing to sell the required fuel under
purchase order attached herewith subject to credit limit;
WHEREAS, the Municipal Budget Officer certifies the availability of funds to
ensure payment thereof;
WHEREAS, the “Procuring Entity” is willing to pay under the condition
stipulated by the “PROPRIETOR” as provided in the receipt:

NOW THEREFORE, for and in consideration of the abovementioned stipulations


the PROPRIETOR AND THE PROCURING ENTITY made this agreement binding
in the spirit of healing and recovery brought by the calamity;
IN WITNESS WHEREOF, we hereby affixed our signature this _____ day of
