INFORMATION

The undersigned Associate City Prosecutor accuses of the crime of Reckless Imprudence
Resulting to Homicide, penalized under the provisions of Article 365 of the Revised Penal Code,
committed as follows:

That on or about the 24 th day of March 2023, in Malolos City Bulacan, Province of Bulacan,
Philippines, and within the jurisdiction of this Honorable Court, the above-named accused,
being then the driver and person-in-charge of a Toyota Fortuner, did then and there willfully,
unlawfully and feloniously drive and operate the same while passing along in the said City, in a
negligent, careless and imprudent manner, without due regard to the traffic laws, rules and
regulations and without taking the necessary precautions to prevent death or injuries to
persons, causing by such negligence, carelessness and imprudence, said automobile driven by
him to crashed into Klay Ibarra’s Honda Civic, which caused the death of the said victim.

is under custody of the law without warrant of arrest has refused to sign a written waiver of his

accused.
