AFFIDAVIT

I,TEOFISTO V. EUGENIO JR. of legal age, Filipino, single and a resident


of Poblacion 2, Midsayap, Cotabato, after having been duly sworn in accordance
with law, hereby depose and say:

That I am presently enrolled as 4th Year College student at the Notre


Dame of Midsayap College, Midsayap, Cotabato taking up Bachelor of Science in
Commerce major in Business Administration;

That in my records with the said school, particularly in my Form 137, my


place of birth was recorded therein as Midsayap, Cotabato;

That my true and correct place of birth is “Digos, Davao del Sur”, as
appearing in my Certificate of Live Birth, copy of which is hereto attached as
Annex “A” hereof;

That I hereby respectfully request the Registrar of the Notre Dame of


Midsayap College, Midsayap, Cotabato to cause the correction of my place of
birth from “Midsayap, Cotabato” to “Digos, Davao del Sur”, the latter being the
true and correct one;

That I execute this affidavit freely and voluntarily, in order to attest to the
truth of the foregoing statements and for such other legal intents and purposes
the same may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 3 rd day of


September, 2003 at Midsayap, Cotabato, Philippines.
