Introduction
This learning and assessment workbook introduces you to the different methods of

• Identify three methods of data collection, and give examples of how each is
useful.
• Compare directive and nondirective approaches to interviewing.
• Differentiate closed and open-ended questions, providing examples and
listing advantages and disadvantages of each.

Data Collection
- Is the process of gathering information about a client’s health status. It must
be both systematic and continuous to prevent the omission of significant data
and reflect a client’s changing health status.

QUESTIONS
OBSERVATION:
Observation is gathering data that uses the five senses, including looking, watching, surveying
and appraising.

EXAMINATION:
Examination is obtaining baseline data, obtain data that will help the nurse establish nursing
diagnosis and plan client's care, evaluate the physiologic outcomes of health care.

INTERVIEW:
Interview is a purposeful conversation between the patient and the nurse, it consists of asking
questions designed to elicit subjective data-what the person says about himself or herself.

Provide the definition of the following phases and give an example for what a nurse

Pre-interaction Phase:
Pre-interaction Phase or Prepatory phase, it is before the first meeting of the nurse with the
patient.
In the Pre-interaction Phase, this is where the nurse prepares for the patient on how to
effectively get the trust of the patient so it builds a good nurse-patient relationship.

Introductory Phase:
Introductory Phase, this is first meeting of the nurse with the patient.

nurses should be capable of enabling them to express their concerns by being open-minded and
also be ready in what action a patient should show.

Working Phase:
Working Phase or Maintenance Phase, the nurse and the patient cooperate with each other in terms of
planning a goal to attain the patient health recovery.
As a nurse, assessing the patient's learning needs and enabling them to understand their condition, treating
the patient as a valued individual that includes the patient in the decisions making about their care, addressing
the patient in the way they prefer, respecting their personal space and treating them with respect at all times.

Termination Phase:
Termination Phase or Concluding Phase, Ending the nurse-patient relationship as it successfully obtain the
goal of the nurse that the patient is fully recovered.
We nurses must value the feelings of our patient because we care for them and help them overcome their
situation. With the help of our communication skills it will influence the patient to communicate openly with
others, to god and being hopeful. We nurses must guide our patient’s health beliefs and practices including
their family in order to necessarily attain healthcare.

Communication
- Exchanging information, thoughts, and feelings among people using speech
or other means. The patient conveys their fears and concerns to their nurse to
help them make a correct diagnosis. Thus, the benefits of nurses being able to
communicate effectively are overwhelmingly critical.

1. What scenario/s have you encountered in your life, that you could have wished
you had known this topic? Elaborate your example.
In my high school days because I'm shy on talking to other people as it
bothers me on how I can communicate with them that they can feel
entertained by me talking to them and they can't feel boredom when talking
to me.

2. How could you have used your knowledge in this/these scenario/s?


This topic will be very useful as it talking about the phases on how we use
our communication skills to gain trust and to build a nurse-patient
relationship. With these strategies it will set a goal for the patient's recovery
and we nurses shall assist them in doing so.

3. As an Augustinian Nursing Student, how would this topic be beneficial to your


career path?
As an Augustinian Nursing Student, this topic would be very beneficial as it
helps us nurses striving to become sensitive that make us more genuine,
which encourages self-growth and self-actualization in both us nurses and
the patient that can increase confidence in us nurses in managing
challenging situations.
