This scenario assignment is part of the Course 5 learner evaluation along with
the clinical evaluation and reflection.
 Learners read the scenario below, answer the questions, and apply the
CARE principles to the scenario.
 This written assignment should be no longer than one page.
 This assignment is graded as complete/incomplete.

Scenario
The continuing care centre has four health care aides (HCA) assigned to wing B.
Annalise has been assigned four clients; one is Mr. Smith, who has been reported
to be combative and at risk for falls. Annalise is on break when Sara, a co-
worker, walks past Mr. Smith’s room. Mr. Smith is trying to get out of bed. Sara is
late in assisting with medications, and she also needs to feed Mrs. Kim because
she missed her lunch. In applying the CARE principles answer the following
questions.
 Explain how the four CARE principles apply to this scenario.
 Identify what Sara should do first and then prioritize the remaining

Answer Key
Compassionate Caring through Competence: Compassion is caring about
another person’s needs. In this scenario, Sara can show concern by ensuring the
clients’ safety. She should go to the client’s bedside, help them meet their needs
(e.g., toileting), and protect them from falling to prevent an injury (e.g., side rails
up, providing pillows for comfort) that may occur such as a head injury, broken
bones, bleeding from a wound caused by the fall, bruising, or skin tears.

Accurate Observations: Sara should respond to the situation as it is happening.


She should use her observation skills (e.g., is he experiencing pain) when
walking past Mr. Smith’s room to further investigate immediately. Sara should
not ignore the client’s needs even though the client was assigned to another
HCA.

Respond: Sara is a member of the collaborative care team, so she knows that
her responsibility is the overall care and safety of all clients on the unit and
ensuring client comfort. Sara should communicate with the assigned HCA,
Annalise, afterwards about what care she provided for Mr. Smith and what
precautions she took to prevent Mr. Smith from injuring himself. Both Sara and
the assigned HCA, Annalise, must document what occurred; Sara should
document the exact events using narrative charting, and Annalise should
document the care she provided afterwards.

Communication also includes communicating to the RN or LPN who is working


with the HCA, to the charge nurse, and to the staff coming on to the next shift.
This incorporates recording and reporting, which allows for continuity of care.

Enhance Quality of Life: By assisting the client, Sara would ensure the client is
feeling supported and that their needs are met. The client may have been trying
to get out of bed for a specific reason and unable to communicate to the staff by
using the call bell. The HCA might assist with toileting or other care needs as
required. This would ensure the client feels content and their needs are met; this
also shows support of members of the care team.
