Side effects of antipsychotic medications (PS CAT DOG)

 P - Photosensitivity
o X direct sunlight
o Use sunglasses, SPF 25, umbrella
 S - Sedation
o Caution in driving
 C - Constipation
o Cholinergic effect (SNS)
o Increase OFI
o Increase fiber diet
 A - Agranulocytosis
o Decreased WBCs → Risk for Infection
o WOF this in patients taking Clozapine
o Instruct patient to report of fever and sore throat (S/Sx of infection)
 T - Tooth decay
 D - Dry mouth
o Cholinergic effect (SNS)
o Give sugarless gum, hard candy to stimulate anticholinergic effects
 O - Orthostatic hypotension
o Dangle feet before arising from bed
o Change positions gradually 
 G - Galactorrhea
o Milk discharge from patients
o Use cotton underwear
 EPS (Extrapyramidal syndrome)
o Cause decreased dopamine due to antipsychotics given
o 3 components (DAP)
 D - Dystonia (DOT)
 D - Dysphagia (difficulty swallowing), drooling
 O - Oculogyric crisis (Upward  deviation of eye)
 T - Torticollis (Stiff neck)
 Tx (ABC)
o Akineton (Biperiden)
o Benadryl (Diphenhydramine)
o Cogentin (Benztropine)
 A - Akathisia
o Restlessness, inability to sit still
o Tx: Propranolol (Beta-Blocker)
 P - Pseudoparkinsonism
o Resting tremors, unstable gait
o Tx: Pramipexole (Mirapex), Amantadine
(Symmetrel)
 What would be the immediate nursing action if the
patient is experiencing dysphagia, torticollis and tremors?
Notify MD.
o X discontinue → relapse of S/Sx
 Other management. Lower the dose/shift to another med
 Prevention: Early detection (notify MD immediately)
 Neuroleptic malignant syndrome 
o Muscle spasms + fever 
o Muscle spasms → laryngospasms → airway obstruction 
o Most fatal SE of antipsychotic medications 
o Immediate nursing action: discontinue immediately 
o Management: muscle relaxants 
o Prevention: hydration 
 Tardive dyskinesia 
o Last SE to occur - would only occur after 6 months 
o permanent, irreversible 
o s/sx: tongue protrusion and twisting, teeth grinding and lip smacking 
o Immediate nurse action: notify MD
o Tx: valbenazine (Ingressa)
