GREETINGS!

I am MELANIE AURIE R. AREVALO, a permanent non-teaching personnel working as Disbursing


Officer I at QUIPAYO NATIONAL HIGH SCHOOL for five years. I respectfully ask for permission to avail the
study leave with pay under Civil Service Commission (CSC) Memorandum Circular No. 21, s. 2004, otherwise
known as “Amendment to Section 68 of CSC MC No. 14, s. 1999 relative the Guidelines on Study Leave” covering
qualified officials and employees. The purpose for my request is to prepare for the November 2020-2021 BAR
Examination which will be held on the four Sundays of November 2021.

I graduated JURIS DOCTOR at University of Nueva Caceres on the 7th of May, 2020. At present, I am
currently enrolled in Lex Scholarium Review Center.

Hence, I have the honor to request permission for six (6) months official leave with pay which is from
June 2021 up to November 2021. I would like to take this opportunity to pursue my dream to become a lawyer that
matters and can be of service to those who have less in life. I understand that this could leave the agency in a
difficult situation but I will see to it that all pending transactions in our school are settled before I start my leave. I
shall resume to work on December 1, 2021.

It is my fervent hope and prayer that this humble request will be given meritorious favor from your good
office.
