I. PROJECT DESCRIPTION

Number of Beneficiaries: 54 Pupils and 2 Teachers including TIC
Location of Beneficiaries: Rajah Sumonsang Dilangalen Elementary School
Date of Implementation: February-March, 2022
Area of project implementation: Sitio Masalicampo, Barangay Kibia, Matalam, Cotabato
Budget Requirement: P38,000.00
Source of Funds: Special Education Fund (SEF)

II. BACKGROUND/SITUATION ANALYSIS

Rajah Sumonsang Dilangalen Elementary School is one of the small schools in Matalam
Central District with a population of only 54 pupils with two (2) teachers. One multi grade teacher
and a Teacher-In-Charge. The school has a total land area of 5,000 square meters. The school
belongs to last mile schools and is still developing. Currently, it has very minimal resources with
three (3) classrooms, one unit building with two (2) classrooms from DepEd and an additional one
classroom was given to us by the MLGU through SEF but the floor needs to be tiled and there is a
need to improve the facilities like putting up of water source, electrical wirings, entrance and exit
routes for the learners to avoid direct contact with each other during the expanded face to face
classes, putting up of isolation and clinic room and purchase of galloons, water jugs, and water tubs
for the learners.

As one of the priority program and project of our Principal-In-Charge Mr. Edgar A. Gatchalian
of Matalam Central District, he advised all the school heads to make a proposal for the improvement
of our school particularly in line with the preparation for the validation of the schools for expanded
face to face classes. He suggested that, since the validation of schools is on its way, provision of
water system shall then be given attention aside from the provision of bond papers, ink, and
printers for the learning modules.

The budget will be taken from the office of the Mayor through the Special Education Fund
allotments. Hence, this proposal was done to seek the approval of the mayor and materialize the
project.

III. Objectives:

The following objectives should be followed strictly in accordance:


1. draft and submit proposal to the office of the mayor;
2. utilize the fund immediately upon receive of the cash or materials;
3. follow the program of works and budgetary requirement of the project.

VI.Budgetary Requirement

Budgetary Requirements and Program of Work will be done by the Mayor’s Office however a
proposed draft of budgetary is attached herewith.

VIII. Monitoring and Evaluation

Monitoring and evaluation of the project will be done before, during and after the
implementation by the Teacher-In-Charge Zacaria L. Kandalayang and Principal-In-Charge Mr. Edgar
A. Gatchalian of Matalam Central District.
