A blood film or peripheral blood smear is a blood test that gives information
about the number and shape of blood cells.
Aim of blood smear
• Blood films are usually examined to investigate hematological problems
(disorders of the blood) and, occasionally, to look for parasites within the blood
such as malaria and filaria.
• Examination of thin blood films is important in the investigation and
management of anemia, infections, and other conditions which produce
changes in the appearance of blood cells and differential white cell count.
• A blood film report can provide rapidly and at low cost, useful information
about a patient’s condition.
The peripheral blood film ( PBF ) is of two types ;
1. Thick blood smear.
2. Thin blood smear.
three basic steps to make the blood film:
1-Preparation of blood smear.
2- Fixation of blood smear.
3- Staining of blood smear.
1-Preparation of blood smear.
We can collect blood sample by
1. Venipuncture ( using syringe )
2. Finger puncture ( using lancet )
3. Heel puncture ( using lancet )
Materials required
1- Capillary blood
2- Glass slide
3- Microscope
4- Alcohol
5- Lancet
6- Leishman's stain
Procedure of blood film
1- Place a drop of blood 1 cm from one end of slide.
2- Place the smooth clean edge of a second (spreader) slide on
the specimen slide, just in front of the blood drop.
3- Hold the spreader slide at a 30°- 45 angle, and draw it back
against the drop of blood.
4- Allow the blood to spread almost to the edges of the slide
5- Push the spread forward with smooth speed.
6- Label one edge with patient ID.
7- The slides should be allow to dry.
Fixation and Staining the slide
1) Leishman stains are prepared in methyl alcohol (Methanol) so that they
combine fixation and staining.
2) Cover the slide with concentrated Leishman stain for about 2-3 min.
3) Diluted the slide with DW for 5 min. Wash with tap water.
4) Leave to dry.
5) Examine under oil immersion lens of light microscope.
Characteristics of a Good Smear
• Thick at one end, thinning out to a smooth rounded feather edge.
• Should occupy 2/3 of the total slide area.
• Should not touch any edge of the slide.
• Should be margin free, except for point of application.

• slide 1 - perfect smear
• slide 2 - smear technique interrupted in middle
• slide 3 - smear was skewed
• slide 4 - blood droplet too thick
• slide 5 - smear too short
