SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

That I, Name, of legal age, Filipino, single, and a resident of address, do hereby
constitute and appoint Name, of legal age, Filipino, married, and a resident of address to be
my true and lawful attorney-in-fact for and in my name, place and stead, and for my own
use and benefit, to do and perform the following act and deed, namely:

1. To represent and execute a Contract of Lease on my behalf over my properties


namely: list all the properties na ipa rent

2. To look for a lessor, negotiate, and process the leasing of my properties namely: list
all the properties na ipa rent

3. To make representation in my behalf with the Bureau of Internal Revenue, Local Civil
Registrar, Registry of Deeds, Philippine Statistics Authority, and other government
agency in processing my TIN, birth certificate, and other government papers and
documents necessary to lease the above-mentioned property.

4. To make representation and to process the payment of all necessary taxes and fees
with the Bureau of Internal Revenue, Local Government Province, Registry of Deeds,
and other government agencies for all the transactions mentioned above.

5. To make representation in my behalf in all my transactions in the Philippines, with


any branch or agency of the Government of the Republic of the Philippines including
Government Owned and Controlled Corporations (GOCC’s), private corporations
and institutions, on which I have transactions as well as its respective departments.

6. To do, sign, pay, settle, negotiate, and execute any obligation, documents and other
writings of whatever nature or kind, with any and all third persons, entities or
concerns, in relation to the aforesaid acts, upon terms and conditions acceptable to
my said attorney-in-fact.

7. To manage, and administer the property above-mentioned to third persons, entities,


or concerns, upon terms and conditions acceptable to my said attorney-in-fact.

8. To sign, receive and provide any money and document in our behalf regarding the
above-mentioned case, and transactions, and do whatever is necessary in order to
implement this special power of attorney.

HEREBY GIVING AND GRANTING upon my aforesaid attorney-in-fact full power and
authority to do and perform any all acts and things pertinent to the foregoing power and
hereby ratifying and confirming all that my aforesaid attorney-in-fact may have done or
cause to be done by virtue of these presents which I may personally do if present.

HEREBY RATIFYING AND CONFIRMING all that my attorney-in-fact or their substitute


shall lawfully do or cause to be done under and by virtue of these presents.
