1. Executive Order or Sangguniang Barangay Ordinance organizing/ reconstituting the BCPC


2. Minutes of Meetings duly signed by the Secretariat head, with attached
- Notices of Meetings with Agenda
- Attendance Sheets
- Pictures (1 minutes per quarter (4 quarters), 2020 meetings)

3. Resolutions Recommending proposed policies, plans and programs for adoption by the
Sangguniang Barangay

4. Approved 2020Annual Work and Financial Plan with


- BCPC Resolution Approving the WFP
- Sangguniang Barangay Resolution Adopting the WFP

5. Barangay Council Ordinance or Barangay Budget providing or appropriating funds for the
BCPC

5. Copy of Barangay Annual Report (LCPC AR Form 001-A)

6. Copy of State of Barangay Address (SOBA) CY 2020 reflecting accomplishments on children

7. Copy of Barangay State of Children Report
