CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This DEED, made and executed this ______________ day of ______________, by and between

_____________________________________ of legal age, Filipino and residing a ____________________


_____________________________________________ and herein after referred to as Member,

AND

ANNA CRISTINA F. VALLEDO, of legal age, married and residing at 3V Heizer St. Balara Filters, Brgy
Pansol, QC and herein after referred to as the ADMIN;

Both the Member and the ADMIN are hereinafter referred collectively as the “Parties”

WITNESSETH:

Whereas, the Member, will receive the payout from the ADMIN and undertakes to pay him as stated in
the Schedule of Payment in this agreement.

Whereas, the ADMIN agrees to give the payout to the Member and on the undertaking of the Member
that the amount received will be paid as stated in the Schedule of Payment in this agreement.

NOW, THEREFORE, for and in consideration of the foregoing, both Parties hereby undertake to
comply with the following terms and conditions to wit:

1. ADMIN, hereby hands and fully delivers, or undertakes to make such delivery in favour of Member

2. MEMBER, hereby agrees and undertakes to pay ADMIN as stated in the Schedule of Payment in this
agreement.

Both MEMBER and ADMIN declare that they have read this document and have fully understood its
contents. Both Parties futher declare that they voluntarily and willingly executed this Agreement with full
knowledge of their rights under the law.
IN WITNESS WHEREOF, the Parties have signed these presents at _____________________ this
