WAIVER

1. I am fully aware that PTC Aviation Training Center Inc., their management, officers, agents,
instructors, or employees will facilitate a generic flight attendant training program consisted of soft
skills training(personality development) and technical skills training(hands on training on safety
emergency procedure, inflight service, firefighting & survival swimming), that PTC Aviation Training
Center Inc., its management and staff will assist in preparing my curriculum vitae(CV) and will assist
in endorsing me to airline companies after completing the whole program(online and face to face
training) but does not guarantee my employment.

2. It is my express intent that this Waiver shall bind the members of my family and spouse, if I am
alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a
RELEASE, WAIVER, DISCHARGE, AND COVENANT NOT TO SUE the above-named
RELEASEES.

IN SIGNING THIS AGREEMENT, I ACKNOWLEDGE AND REPRESENT THAT I have read

the foregoing Wavier, understand it and sign it voluntarily as my own free act and deed; no oral
representations, statements, or inducements, apart from the foregoing written agreement, have
been made; I am at least eighteen (18) years of age and fully competent; and I execute this
Agreement for full, adequate and complete consideration fully intending to be bound by same.

IN WITNESS WHEREOF, I have signed this Waiver and Agreement under seal on this

of my photograph in Marketing and Integrated Brand Promotions of PTC AVIATION
TRAINING CENTER INC which will be released this 2020 onwards. This is for the purpose of
advertising and promotions of PTC AVIATION TRAINING CENTER INC.

The photographs may be made available in social media, flyers, banner-ups and other
collaterals for the aforementioned purposes.

I, therefore, agree that I shall have no claim against PTC AVIATION TRAINING CENTER INC. or
anyone accessing this communication collateral, whether online, in print or by any other
means.

These authorizations shall remain effective for the duration of the promotions, unless
sooner revoked in writing, delivered to and received by PTC AVIATION TRAINING CENTER
INC.
