John Smith presented with a complaint of chest pain that started an hour ago.
He describes the pain as a pressure-like sensation that radiates to his left arm.

He reports feeling short of breath and lightheaded. He has no previous history of


chest pain, but he has a medical history of hypertension, hypercholesterolemia,
type 2 diabetes mellitus, and atrial fibrillation.

Upon examination, blood pressure is 160/100 mmHg, and an ECG reveals


ST-segment elevation in leads II, III, and AVF.

Writing Task

Dear Dr. Michael Johnson,

I am writing to refer John Smith, a 65-year-old male patient who presented with a
complaint of chest pain that started an hour ago. He describes the pain as a

and lightheaded. Upon examination, blood pressure is 160/100 mmHg, and an ECG
