Ambulation, progressive
Objective: To assist a patient with progressive ambulation according to the standard of
care.

Checklist Step Comments

__ Review the patient's medical record for history,


diagnosis, and therapeutic regimen.

__ Gather and prepare the necessary equipment and


supplies.

__ Perform hand hygiene.

__ Confirm the patient's identity using at least two


patient identifiers.

__ Provide privacy.

__ Screen for and assess the patient's pain using


facility-defined criteria.

__ Treat the patient's pain, as needed and ordered.

__ Explain the goal of ambulation to the patient and


family (if appropriate) according to their individual
communication and learning needs.

__ Ask a coworker for assistance, as needed.

__ Remove equipment and other objects from the


patient's path, if appropriate.

__ If the patient has IV tubing or other tubing or


catheters, secure them.

__ Lock the wheels on the bed, if appropriate.

__ Position the bed horizontally and the patient


laterally, facing you.

__ Assist the patient with moving the legs over the


side of the bed.

__ Grasp the patient's shoulders, standing with your


feet apart to form a wide base of support.

__ Help the patient rise slowly. Ask the patient to


help by pushing up from the bed with the arms as
you steadily raise the patient to a sitting position.
Alternatively, raise the head of the bed to a 45-
degree angle.

__ While the patient adjusts to the upright position,


stand facing the patient and observe closely.

__ Check the patient's pulse rate and blood pressure,


as needed.

Helping the patient stand

__ Help the patient put on a robe and nonskid


slippers or socks or shoes, as appropriate.

__ Don't allow anything to dangle around the


patient's feet.

__ If the patient is cooperative and can fully bear


body weight, have the patient place the feet flat
on the floor and let the patient stand
independently. Stand by for safety.

__ Alternatively, if the patient is cooperative and can


partially bear body weight, apply a gait belt and
use another assistive device, as ordered. If you
must lift more than 35 lb (15.8 kg) of the
patient's weight to assist with standing, use a
power stand-assist lift.

__ Encourage the patient to look forward and not at


the floor.

Helping the patient sit

__ Make sure that the chair is secure or the


wheelchair's brakes are engaged.

__ After the patient stands, have the patient pivot


and lower the body into a chair with armrests and

a straight back or into a wheelchair.

__ Instruct the patient to sit with the lower back


against the rear of the chair, feet flat on the floor,
with the hips and knees at right angles and the
upper body straight. Tell the patient to flex the
elbows and rest the forearms on the armrests of
the chair.

__ If you leave the patient sitting in the chair, make


sure that the patient has a call light or other
signal device.

Helping the patient walk

__ If the patient can bear weight fully, have the


patient begin walking while you stand by for
safety.

__ If a gait belt is necessary, place one hand under


the gait belt in back of the patient. If the patient
is weaker on one side, be sure to walk on the
weaker side when ambulating. Encourage the
patient to use an assistive device, if needed.

__ Walk closely behind and slightly to the side of the


patient, keeping the same pattern and pace as
the patient.

__ Give the patient verbal and tactile cues for


encouragement.

__ Allow for rest periods, as needed.

Completing the procedure

__ Reassess and respond to the patient’s pain by


evaluating the response to treatment and
progress toward pain management goals. Assess
for adverse reactions and risk factors for adverse
events.

__ Perform hand hygiene.

__ Put on gloves and, as needed, other personal


protective equipment. 

__ Clean and disinfect reusable equipment according


to the manufacturer's instructions. 

__ Remove and discard your gloves and, if worn,


other personal protective equipment.
