COMES NOW THE PLAINTIFF, unto this Honorable Court, most respectfully
states THAT:

1. The Honorable Court rendered a Decision in favor of the herein plaintiff, the
dispositive portion of which states:

“PREMISES CONSIDERED, the small claims action for sum of


money is hereby GRANTED in favor of the plaintiff and against the
defendants who are directed to pay the amount of Php___________ plus
six percent (6%) interest per annum to be reckoned from __________
until fully paid.

SO ORDERED.

2. The defendants, up to this date, failed to pay the said amount;

3. Due to the above-cited reason, the undersigned respectfully moves that an Order
