AFFIDAVIT OF NO PENDING CASE AND AUTHORIZATION

I, MARY C. TAN, of legal age, Filipino Citizen, married and a resident of


Barangay Camanci Sur, Numancia, Aklan, Philippines, after having been sworn
to in accordance with the law, do hereby depose and state:
1. That I am retiring from government service as Administrative Officer II
of the Provincial Accounting Office, Provincial Government of Aklan,
effective December 29, 2022;
2. That I have no pending criminal, civil or administrative case before
any tribunal, agency, or court in the Republic of the Philippines;
3. That I hereby authorize the Office of the Provincial Accountant to
deduct all my financial obligations with the Provincial Government of
Aklan in computing the proceeds of my terminal leave benefits;
4. That I am executing this Affidavit in connection with my claim for my
terminal leave benefits and for all legal intents and purposes it may
serve.
