1. References:
a. Spot Report from this office with same subject as above dated February 17, 2023 ;
b. Police Blotter Entry No. 2023-02-14, page 40 dated February 17, 2023; and
c. Police Operational Procedure.

2. This pertains to the shooting incident that transpired at about 6:00 AM on


February 17, 2023 at ____________________ that resulted to the death of the
victim __________________ and the suspect was identified as
______________________ who fled on foot to unknown direction after the
incident.

3. Please be informed that hot pursuit operation conducted by the personnel of


___________________ resulted in the arrest of the suspect at about 10:30 AM
of February 17, 2023 at ______________________. The suspect had in his
possession a Cal .45 pistol with Serial No. 3191M9 which was confiscated by
the arresting officers. The suspect was apprised of his constitutional rights and
was brought to ___________ for proper documentation and medical
examination prior inquest proceeding.

4. This station has requested the SOCO and awaiting result of the Ballistic Cross
Matching on the recovered six (6) cal .45 cartridges, one (1) deformed slug and
one (1) cal 45 pistol. IOC - _______________
