PDP Assigments.
I am a hardworking, independent, caring and friendly person who is extremely trustworthy. I believe I have
the attitude and focus that will allow me to flourish in any opportunity that is given to me and I use these
attributes as an effective team player. I am an approachable and responsible person who can stay composed
and collected in high-pressure situations while following instructions and communicating with
other colleagues.
Colleagues / students and staff who know me I have been described as fun, strong willed and with a good
sense of Humour. These numerous qualities have taught me a wide range of abilities that I am currently
looking to use to and advance within such a new career role while aiming for outstanding performance in
every aspect of my profession. Being such a competent and outgoing person, I understand the value of
performing as part of a team as well as on my own, since I believe it is one of the most important aspects of
working.
I work excellently in different situations all in a non-judgmental manner, I can adapt to most environments
comfortably. My positive outlook on life contributes to a good team spirit in any environment. I hope to
leave a lasting impression on the individuals and groups I work with and the people I work alongside.
I joined health and social care course for a personal reason as someone close to me was diagnosed with
schizophrenia at the age of 18 I became a full-time carer as her condition worsened this course can provide
me with a wider range of understanding of health and social care practices, policies, and systems, as well as
skills in communication, problem-solving, and teamwork. Additionally, studying health and social care may
lead myself into career in a related field, such as social work, nursing, or counselling working with mental
health patients. Joining this course will allow me to have a positive influence on the lives of other people.
Whether I am helping to improve their physical health, mental well-being or maintain their safety. This
course will allow me expand my knowledge and skills as I have already had 2 years of experience with my
own family member. This course will also enable me to improving someone's life is extremely rewarding. No
two days will be the same. Each day will be different, making it a flexible and interesting job role.
From the above, I can confidently say that I demonstrate and can apply communication skills, team working
and leading, empathy, time management and organisational skills confidently daily through practice in
college and work experience opportunities. I apply time management and organisational every day when
managing assignment loads and how I always get them in on time while achieving all the criteria across all of
them. The same skills as well as team working and leading, I show confidently through my personal job in a
care, college during group activities and even in the past work experience opportunities in a hospitals,
Pharmacy and SEN school. I am also confident in active listening and the ability to act on my own initiative
and attention to detail as I try to remain as independent when I can, of course, if suitable and I am a good
listener which helps me apply the rest of the above skills very well, since they work hand in hand and enable
me to problem solve. Through work experience I also learnt to actually be more empathetic and
understanding too as well actually show it to service users and people. However, I do need to work on my
confidence and my socialisation area of things as sometimes I do get intimidated, stressed and easy
overwhelmed with social situations, therefore my stress management and emotional control, as well
confidence needs some work.
I also work part time, and this also helps me build up essential health and social care skills, attributes and
qualities essential for all types of professionals. I have worked with children in the past too, as a part time
job. My current and old job combined allow me to work with people of varying needs and preferences and
wishes. I also have to work in a team and this allows me to build on my team working abilities and skills, as
well as my leadership ones because I also used to lead groups of children and worked independently to
complete the tasks within my role. Furthermore, both these roles required me to be able to act on my own
initiative which I do often when needed which is also another skill of mine. On top of this, working has
allowed me to develop essential social skills and improved my communication as well as confidence since I
worked with people all the time and met them kindly, assisted their needs and supported them.
The skills and qualities and attributes listed are essential and all of these things combined have helped me
develop and better the skills, attributes and qualities I need to be a successful individual. Having awareness
on my future role through research and practicing and developing all the skills, attributes and qualities
required are important for many reasons.

My first long term target is to graduate as a registered Health and social care professional in 2025. I will
measure this target by continuously getting feedback and reflecting on it alongside my mentors so I can
assess my own progress and be certain of what track I am on and if I am on the right pathway to successfully
graduating. Getting feedback during my placement will also allow me to measure my progress because I can
reflect on what I can do to improve my midwifery practice and identify development within my practice, so I
can be competent enough to register as an HSC professional I feel like this target is achievable because I am
confident about my abilities and the challenges within health and social care intrigue me and motivate me
further, rather than put me off.
Additionally, this target is realistic because almost everyone graduates after completing their 3-year health
and social care course, and I am confident I will be one of them in 2025. In order to graduate I will use my
academic and practice skills and knowledge and consistently check in with what I need to do to remain on
track of all my required hours and theory. Using my time management and organisational skills will aid me in
this because they will allow me to balance my home, personal and university and work experience life in a
healthy way. A strength is that I have been assured that I will have all the support and help I need to
graduate so I will work with my tutors and lecturers and placement officers in university to achieve this.
Another strength of this target is that if I achieve this, in turn I gain everything that I need to start working in
the health and social care field and fulfilling the dream career I have always wanted. I am also confident
about the support I will receive from my mentors while on the course because the interviews I have had
have assured me I will be well supported and enabled to become a competent and high quality, ideal health
and social care professional.
My second long term target follows on from my previous target of graduating as a registered HSC
professional, because my second target is to be working or find a job during/right after university so I can
work as a mental health care professional and fulfil my career aspirations. Ideally, I wish to start working
right after I graduate, so I want a job lined up for myself by 2025. This target is measurable because during
university, I must be given numerous opportunities where I can apply and gain my experience in various
hospitals and setting. This target is also realistic because after graduating, people go onto working and
maintaining the careers they have worked for, by taking on job opportunities offered or going onto other
degrees etc. UCAS also states a high number or graduates that go onto working right after studying too
which encourages me and brings up my confidence of achieving this target. I am going to get a job as a
mental health professional by ensuring that I complete all the work I need to have done in order to graduate.
The skills and knowledge I develop through hsc experiences and placements and through the theory side will
aid me to deliver high quality maternal care competently. My confidence, compassion, communication,
understanding and other skills will allow me to work as a mental health care professional as well
as successfully maintain my role, as well as my understanding and caring attitude, ability to get on well with
people from a wide range of backgrounds, emotional and mental strength, good observation, ability to act
on own initiative, patience, maturity and willingness to take responsibility/accountability. Alongside this
long-term target, I would like to be independently be financially stable with a good house and stable work.
My target is to be working or find a job during/right after university so I can work as a mental health worker
and fulfil my career aspirations. Ideally, I wish to start working right after I graduate, so I want a job lined up
for myself by 2025. Though I am yet to achieve this long-term goal, I have set a deadline of about 3 years'
time. I am determined to look for a job in the health and social care field, more in the direction of mental
health alone either as a health care assistant or a social worker. This target will enable me to push my
abilities and skills to make sure I pursue my dream job as well as fulfilling all my aspirations and goals within
this time frame.
Moreover, achieving this goal will provide me with financial stability with a good income and house, which
will significantly impact my life positively. I am optimistic about the future and the realization of my goals. By
setting achievable targets and working hard towards them, I am confident that I will fulfil my aspiration of
being a mental health worker, and this will not only fulfil my dreams but also enable me to help others in
need.
My other target is to graduate as a registered health and social care worker in 2025. In order to achieve
this long-term goal, I plan to complete my current course by then. However, I understand that there is
still much more to learn beyond this course. As such, I am considering pursuing a postgraduate degree in
either health and social care or another medical field. By doing so, I believe that I will gain valuable
insights into my chosen field, which will provide me with more opportunities to pursue different career
paths.
In the short term, I intend to focus on completing the first year of my current course and handing in all of
my outstanding assignments. This will be a challenging task, as working in the medical field can often be
overwhelming and stressful. However, I am confident that I can rise to the challenge and meet my
objectives.
By setting these two targets, I am able to work towards my long-term goal while also achieving smaller,
more immediate objectives. This approach allows me to develop my skills and knowledge gradually over
time, while also building my confidence and resilience in the face of challenges. Ultimately, I believe that
this will enable me to achieve my long-term goal of becoming a registered health and social care worker
by 2025.
