Community Health Nursing


Day 1

The first day of the Community Health Nursing exposure and I belong
in Group 13 is composed of 12 students from Section 1D and 1E which is
being handled by Mr. John Lenard Camilo as our CHN Clinical Instructor. Our
Group is assigned to be exposed to Brgy. Magdum, Tagum City, Davao del
Norte.

Our call time was announced to be 7:30 in the morning at the TDCI
school ground. The group did a prayer and waited for the remaining students
who haven’t yet arrived. After 15 minutes of waiting, we took off by 7:45 to
proceed to our assigned barangay.

We arrived at the Barangay Hall of Magdum and right next to it is their


Barangay Health Center. We entered the Barangay Hall and got oriented
about today’s exposure while waiting for the Barangay Captain and the
Barangay Nurse who went out to retrieved some vaccination. We were
instructed to proceed to the second floor of the Barangay Hall where their
Teen Tambayan is located. We were said to wait there until the Barangay
Captain and Barangay Nurse arrives. After some minutes, the Barangay
Captain could not show up but the Barangay Nurse arrived so we wet
downstairs where their Health Center is.

Before heading down, Mr. Camilo selected 4 students to be at the front


desk, 3 students at the Nurse’s office to assist and instructed the remaining
students to stay put until further notice. I was assigned at the Nurse’s office
where we observed and performed immunization to babies.

For details, the baby I was about to immunize should get 3


vaccinations. One in the right leg and two in the left leg. I first observed the
Nurse to aspirate the Penta vaccine. The nurse aspirates 1.5ml and directed it
to the baby’s right leg. Next is the Polio vaccine which will be injected in the
baby’s left leg. For the polio vaccine, the dose in the vial was limited and
should be aspirated in a certain amount that is why sir Camilo aspirated the
dose instead for me. He then gave the syringe and assisted me to pinch the
baby’s leg and pointed the location where I should insert the needle. My first
immunization attempt was a successful except for the part where I did not put
the cotton ball in the baby’s leg right after I pulled out the syringe. After that,
we were asked to go outside and call two students who were told to stay put
to have their turn to assists the Nurse inside the Nurse’s office.

After that I wandered around the Barangay Health Center to take some
pictures and eventually ended up in the front desk where some of the
students were assigned. The BHW in the front desk also taught us so many
things. Some of it was to log the walk-in clients’ appointments, how to operate
their automatic BP equipment, taught us how the Injectable Calendar works
where it is indicated when the client is suppose to come back have their Vital
signs retaken, taught us how the height and weighing scale works, and let us
take the baby’s weight using a weighing scale for babies also to take the
baby’s respiratory rate. I was called once again to the nurse’s office to have a
second try in immunizing the baby. This time I was not nervous at all and
actually did better than my first and did the injecting properly.

After each one of us tried the task one by one by switching places of
assignments, we stayed in the front desk of the Barangay Health Center
where the Nurse said few words. He introduced himself as Charlu Pasculado,
the Barangay Nurse of Madaum. He oriented us about the health services
they offer at every day of the week. He also explained further the details of the
babies’ immunization and how the paperworks and forms works as well as
thanking us for today’s assistance. He then called the Barangay Nutrition
Scholar who intruduced herself as Liza Sunido. She also gave a few and
further explanation of their barangay’s services mainly about the issues about
the malnutrition. She mentioned the Barangay’s planned program which is the
Nutrition Month to be held this July. After that, the BHW president came out
and introduced herself as Rama Asube. She also talked about the services
they offer and talked about the equipments their health center have. She also
talked about the history and achievements as well as sonsorship of their
health center and appreciated the Barangay Captain for the immediate
assisstance and response whenever they have any lackings or shortage of
medication in the health center. She then introduced each of the BHW
members that is working in the health center.

After that, we thanked the Nurse and the rest of the workers and went
out for morning dismissal and instructed to be at the school by 1PM. For the
afternoon, sir Camilo explained the IDB form and stated examples of what
questions to ask and what factors to note for our house-to-house interview.
He also explained FNCP and the Family Coping Index which will be done
individually for weekly outputs. After the discussion, we were dismissed for
the day.

Today, I learned all the things that we were taught from operating the
equipment to immunizing babies as well as reading their previous
immunization data. The moment we were instructed to have a try at
immunizing the baby was nerve-wracking because all we did in our Return
Demo is how to do the IM to the dummy and what was more nerving is that
we get to do it to a baby who is very fragile and vulnerable. But, after trying
once, It was actually not hard at all because we were guided by the nurse and
our Clinical Instructor. After having my second try, that was when I can say
that it was doable but of course, I still need some guidance as I was not yet an
expert and authorized at this matter. I had fun today and I am honored to get
to do an actual immunization. The Nurse and the BHWs were very kind and
accommodating. I am grateful for having this experience.
