Weekly Check in Form:

Please fill this form out each week and answer all the questions with complete
honesty.
Please send your check in photos on the app to me on the same day each week
(as well as all measurements), my check in days for clients run on a Wednesday
or Sunday.
Please wear the same clothing each week, for example shorts and a top,
underwear, or very tight clothing. (This is so I can see your physique change
and so I can collect accurate data)

Please ensure not to eat or drink anything before weighing yourself, taking the
photos and taking your measurements, this will ensure we are getting accurate
data each week.
We will be exploring your self-worth and stress levels in this check in, this is a
safe space for you to inform me of any problems you deal with in the week, it
does not matter how big or small the problem may be, I am here to help you in
all aspects of life.
