Ambulance Report

The OIC of Siruma Municipal Hospital wrote a letter to the Provincial General Services Officer requesting inspection, maintenance, and repair of their ambulance with conduction sticker number GB 6568. According to reports from their ambulance drivers, the ambulance has been experiencing minimal engine oil leaking, unusual noises from under the chassis, and worsening issues since its last reported problems in August 2020. Dear Sir,

Good day!

Last September 16, I relayed to you the report submitted by our ambulance drivers, thru text
message, regarding the findings during their Routine Maintenance Check-up of our ambulance with
Conduction Sticker No. GB 6568. According to their report, there was minimal engine oil leaking at
the muffler, and an unusual noise was observed from the under chassis area, which are considered
abnormal in engine operations. It was deemed managhgeable at that time, nevertheless, was still
reported. After several ambulance conductions since then, worsening of the said reports were noted
and was handled with cautious driving.

The said ambulance, is the same ambulance reported and submitted for inspection, maintenance,
and repair to GSO last August 2020.

In line with this, I would like to request thru your office, for the said unit to be thoroughly checked
and repaired if necessary, and if possible, a temporary replacement unit/ vehicle be given to
continue our ambulance service for the people of Siruma.

The safety of our patients and employees are of utmost importance.
