The Space Shuttle Disaster

The Space Shuttle Challenger exploded 73 seconds after takeoff on January 28, 1986, killing all seven astronauts aboard. It exploded because an O-ring, which was used to secure sections of the solid rocket booster, failed due to the cold temperatures that morning. The O-rings were not designed to handle such cold and had only been tested at higher temperatures. Engineers warned NASA management that it was not safe to launch under those conditions, but their warnings were ignored, leading to the tragic accident.
