1. Reference: Individual Performance Evaluation Report, 1 st Semester CY


2023.

2. This pertains to the performance evaluation report of PLT HELEN C


SALVADOR by the undersigned for the period of January 1 to June 30, 2023. The
undersigned evaluated and rated said personnel with Outstanding (OS) for the
following reasons:

a. Said PCO showed consistent exemplary performance of service in her


respective duties and positions since her assignment to HRDS;

b. Her inimitable work, ethic, competence, and dedication to uplift the


image of the HPG contributed positively to our Unit Performance
Evaluation Rating; and

c. The undersigned can personally vouch for her character, dependability,


and right attitude in approaching and managing job-related activities.

3. In this regard, the undersigned respectfully requests the approval of my


recommendation to the above-mentioned PCO with an OS rating.
