RELEASE, WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, _________________, Filipino citizen, of legal age, and a resident of


_______________________________, on oath, say that I have ceased to be employed by
XX. (the “Company”) on ______________ due to resignation. Furthermore, I
acknowledge receipt of the total amount of _____________ in full satisfaction of all
employment benefits due me, including but not limited to salaries, 13 th month pay,
retirement, redundancy, separation benefits, if any, from the Company arising from my
employment and the cessation of such employment. I acknowledge receipt of all
separation benefits due me from the Company, its affiliates, officers, directors or assigns
and hereby waive and relinquish all rights that I may have to any further amounts that
may be accorded as separation benefits. In consideration for this amount:

1. I hereby release and forever discharge the Company, its affiliates, its officers,
directors, agents or other parties with which the Company does business, from any
claim, demands or complaint, for lack of cause of action therefor and hereby withdraw
any claim or lawsuit if I have any, now or in the future, against them under any
contract, statute or local Company policy whatsoever relative to my employment with
the Company and the cessation of such employment. Any consideration received by me
shall not be taken by me or my heirs/assigns as an admission of liability by the
Company for all legal intents and purposes.

2. In addition, I acknowledge that during my employment with the Company, I


received all compensation and benefits which are due me under law, contract or
Company business practice. Should I be found, judicially or otherwise, to be entitled to
any further amount, I admit and acknowledge the aforementioned consideration as full,
complete and final satisfaction of any and all such undisclosed claims. I also
acknowledge that I have not been and am not now suffering from any work-connected
illness or disability.

3. I agree and obligate myself to keep this matter, the receipt of the above amount
from the Company, and all matters made known to me by reason of my employment in
strictest confidence and that no unauthorized disclosure or reproduction of the same
shall be made by me at any time.

4. I have read this document and the release and quitclaim executed herein is
given by me willingly and voluntarily and that this document is a complete settlement
of all my claims against the Company, its officers, directors and agents.
