Narrative of Incident:
It was so happen that the concern child has drastically committing numerous days
of absent in the first month to second month of this School Year especially during afternoon
session. It really affect his school performance in both academic and non- academic points,
where it may lead for him to have failed their study or the worst for dropping out if this will
not be resolved earlier and as much as possible.
That is why this very significant home visitation being conducted to make some up
and catch up to prevent from any failing grades and others.

Action Taken:
Send a letter to the mother, asking permission for me to visit their house and have a
heart to heart talk regarding the case of her son.
Aside from suffering too much toothache, the child looks for a job to help his parents in
their financial needs at times.
We both agreed that as a teacher will still accept the child even if he could always
absent. And yet the teacher insisted to have a special and additional remediation for those
lessons that the child left from being absent to help him cope-up with the lessons missed.
The parents will see to it that her son will be in the school even thrice a week so that he will
not be totally loss from his lesson. Her mother will make all what she can do to let her son
be in the school always.
