Employee Work Evaluation Form

This document is an employee evaluation form for Campus Recreation used to review employee performance based on six criteria: skills, passion, initiative, reliability, interaction, and trainability. The evaluator is asked to rate the employee on a scale of 1 to 5 in each category, with 1 being the lowest and 5 being the highest. Space is provided for qualitative comments on strengths and areas for improvement in each area. The form also collects information on whether the employee will receive a wage increase and, if so, the effective date.
