APPLICATION FOR COMMUNITY SERVICE


ACCUSED, through the undersigned counsel and unto this Honorable
Court, respectfully applies for the imposition of community service in lieu of
service in jail, pursuant to the provisions of Republic Act no. 11362 (R.A. 11362)
and A.M. No. 20-06-14-SC (Guidelines in the Imposition of Community Service as
Penalty)

In support of this application, accused states:

1. That accused-applicant is of legal age and residing -


________________. On July 7, 2023, accused pleaded guilty to the
lesser offense of violation of Section 3(a) of Republic Act No. 9287,
amending PD No. 1602.
2. The Honorable Court sentenced the accused to suffer one the
straight penalty of imprisonment of thirty (30) days.
3. Accused-applicant submits that she has not previously availed of the
benefits of R.A. 11362 and no appeal has been perfected as of the
filing of this application.
4. Accused-applicant is physically able and willing to comply with the
terms of the community service program that this court may deem fit
for her and undergo rehabilitative counseling.
5. That the grant of his application will not depreciate the gravity of the
crime charged nor cause any undue risk that during the period of
community service, accused-applicant will commit another crime.

PRAYER

WHEREFORE, premises considered, it is respectfully prayed of this


Honorable Court that the Application for Community Service be approved.

Other reliefs just and equitable under the premises are likewise sought.
