Respectfully returned to Mr. Marvin G. Cañetan and Mr. Roldan Francis D. Alagano
through Officer-in-Charge Juanilla R. Premne the herein request for Relief from Accountability
without action pending the submission of the following documents:

1. Report of Lost, Stolen, Damaged, Destroyed Property (RLSDDP) (Appendix 75,


GAM Volume II)
2. Affidavit or Sworn Statement of the proper accountable officer on the facts and
circumstances surrounding the said loss, supported by the Affidavit of two (2)
disinterested persons who have personal knowledge of such fact of loss;
3. Final police report showing the steps taken by the police authorities to recover the
property lost and to apprehend the suspect(s) and the present status of the case;
4. Description including the book value, date of acquisition, property number, account
classification, condition of the property, and other relevant information of the
properties lost as attested by the concerned officials as the case may be;
5. Latest inventory and inspection report preceding the loss;
6. Certified true copy of the Property Acknowledgment Receipt for the HP Pro Book
445G6 Laptop;
7. Other relevant documents such as:
a. Certified true copy of Transmittal Slips and/or Acknowledgment Receipts
from Philippine Statistics Authority (PSA) Cebu Provincial Statistical
Office to the Local Government Unit (LGU), Registration Center
Supervisors, and Registration Kit Operator;
b. Service Contracts of the concerned Contract of Service Workers;
c. Memorandum of Agreement between the PSA and LGU;

d. Memorandum of Agreement between the LGU and Gaisano Mactan Island
Mall;
e. Information as to whether or not the premises of the Registration Center are
manned by security guards. If so, the respective Sworn Statements or
Affidavits of the guards respecting the incident should be obtained and
submitted;
f. Authenticated picture(s) taken relative to the theft incident.
