2 Which version and edition of Oracle DB do you have? (e.g., 12c Enterprise Edition)
3 Which Weblogic version is used in the Forms? (e.g., 12c Forms/Reports Dedicated license)
4 How do clients access Forms? (e.g., JWS, FSAL, Applet, other)
5 If the access to Forms is browser-based, are there any specific browser requirements for client
browsers?
6 Are there any other requirements for clients? (e.g., operating system, registry, screen resolution,
installation, other)
7 Are the client users in-house or remote/multi-site? How do the multi-site users connect?
8 Is there a one set of Forms that all users use or does each customer have their own local set?
9 If local sets are used, are all customers currently on the same version set?
10 If local sets are used, are there any customizations that one customer has that the others don't?

local apps, etc.)
14 Do you use a client interaction with the local machine? (e.g., reading/writing files locally, registry, HOST
commands, OLE2, terminal/IP logging, etc.)
15 Do you use any web service endpoints? (e.g., REST/SOAP)
16 Are there any scheduled jobs running on the DB?
17 How are Forms users authenticated? (e.g., LDAP, Single-Sign-On, DB Users, other)
18 How do Forms users collect allocated roles? (e.g., DB Roles, table-based, other)
19 Does the Forms application make excessive use of the POST command? (e.g, navigating through
several screens before ultimately clicking Save/Cancel)
20 Do the Datablocks generally modify the underlying object or is there an API involved? (please detail)
21 We often find that a few Oracle Forms share similarities that can be combined into a single Oracle
APEX screen. Can any of these be identified in your case?
