We would like to ask your permission to allow our team to conduct a survey in the Cavite

Provincial Jail. This is in light of our thesis entitled “Satisfaction of the Persons

Deprived of Liberty on Therapeutic Community Modality Program Implemented in

Trece Martires City Provincial Jail: Its Implication to Mental Health Management”

Our group will be conducting the survey among the persons deprived of liberty (PDL)

that are situating in your facility.

The respondent’s participation in the conduct of study will be voluntary. All information

provided will be treated with utmost confidentiality and it will only be use for academic

purposes. Once the data that we acquired have been analyze, we will provide a copy in

greater detail for your institution if you would be interested.


If you agree, kindly sign below acknowledging your consent and permission for us to

conduct this study at the facility and return the signed form on an enclosed envelope.

Your approval to conduct this study will be highly appreciated. Thank you in advance for

your interest and assistance with the completion of this research.

The undersigned students are currently conducting their undergraduate thesis at Trece

Martires City College in compliance with the requirements of their program under the

Bachelor of Science in Criminology. The researchers with the supervision of their research

adviser are now working on their thesis title entitled, SATISFACTION OF THE PERSONS

DEPRIVED OF LIBERTY ON THERAPEUTIC COMMUNITY MODALITY

PROGRAM IMPLEMENTED IN TRECE MARTIRES CITY PROVINCIAL JAIL: IT’S

IMPLICATION TO MENTAL HEALTH MANAGEMENT.

In this regard, we are humbly requesting your good office to provide us with the following

data:

Total strength (number) of persons deprived of liberty that are currently confined in your

institution. (Also, the researchers are hereby seeking your consent and approval to allow the

PDL’s to be their respondents and to participate in data gathering procedure by answering

the provided survey questionnaire); and

The Therapeutic Community Modality Program being implemented in your institution

If you have any comments or require any additional information and suggestions that would

help them improve their study, please do not hesitate to inform them thru email at

[email protected].
Rest assured that all the information and the findings will be handled with the utmost

confidentiality and it will be used for academic purposes only.
