INFORMED CONSENT FORM

PSYCHOLOGICAL REPORT (THEORIES OF PERSONALITY & ETHICS)

INTRODUCTION

In partial fulfillment of the courses Theories of Personality and Ethics offered by the Department of
Psychology at NU Lipa, Mary Jhune C. Dimaculangan from the Psychology Department, PSY221, is
conducting a personality test and interview.

It is imperative that you comprehend the rationale behind this research and the nature of your
involvement before making a decision to participate. We kindly request that you carefully review the
following information: If there are any aspects that are unclear or if you require additional information,
please do not hesitate to inquire with the researchers.

The primary objective of this test and interview is to generate a comprehensive psychological report and
provide the researcher with an opportunity to gain practical experience in administering a personality test
to a client. This valuable experience will contribute to my development as a future psychometrician by
allowing me to gain early exposure to the requirements and responsibilities associated with this field.

VOLUNTARY PARTICIPATION

Your involvement in this personality test and interview is entirely voluntary, granting you the freedom to
choose whether or not to participate in this assessment. Should you decide to partake, you will be
required to provide your consent by signing a form. However, please note that you retain the autonomy
to withdraw your participation at any time, without the obligation to provide a reason. It is important to
emphasize that withdrawing from this test and interview will have no adverse impact on any existing
relationship you may have with the researcher. In the event that you choose to withdraw prior to the
completion of data collection, your data will be promptly returned to you or securely disposed of through
appropriate measures.

STUDY/TREATMENT PROCEDURES

The administration of the personality test will commence with the presentation of the informed consent
document, which will be followed by a comprehensive explanation regarding the nature and objectives
of the assessment. Clear instructions and relevant reminders will be provided to ensure proper
understanding and guidance throughout the test-taking process. Once the test has been completed, the
researcher may schedule a subsequent appointment with the client to conduct the interview, which serves
as an additional source of information for the development of the psychological report.

It is important to note that there are no imposed limitations or constraints in terms of the duration or
extent of the client's responses during the test.

RISKS AND BENEFITS

Your autonomy and freedom to make choices are highly regarded in this process, and you have the right
to decline answering any or all of the questions posed to you during the interview. Moreover, if at any
point you feel the need to discontinue your participation, you are entitled to do so.

 During the course of the interview, it is possible that you may encounter questions that evoke
unpleasant, disturbing, or otherwise unacceptable emotions. Certain topics may potentially elicit
negative emotional states and impact your mental well-being. It is crucial to be mindful of this
possibility.
By engaging in this assessment, we aim to further explore and evaluate your potential career paths and
your overall attitude towards them. This endeavor will provide valuable insights into your vocational
aspirations and help shape your future endeavors.

CONFIDENTIALITY AND PRIVACY

For the purpose of this test and interview, your comments will not be anonymous. Every effort will be
made by the researchers to preserve your confidentiality including the following:

 Your name would be ask to be write down in the test.


 In making the report, if you are not comfortable, I could put a nickname of your preference.
 Notes from the test, the test itself, interview notes and and any other identifying participant
information in a locked file cabinet in the personal possession of the researcher/s.

 Once the report has been completed, all the data will be deleted immediately.

Participant’s data will be kept confidential except in cases where the researchers are legally obligated
to report specific incidents. These incidents include, but may not be included to, incidents of abuse and
suicide risk.

COMPENSATION OR INCENTIVES

Compensation or incentives will not be necessary, however, upon the discussion of the informed consent
the participant will be ask of his/her liking (e.g. food, school supplies and such).

afterwards as fulfillment to the courses THEORIES OF PERSONALITY AND ETHICS.
