AFFIDAVIT

I, Dr Naveen Kumar S/o. M.Srinivasan, aged 27 years residence of Door no. 1/57,
BairamangalamVillage and post, Denkanikottai Taluk, Krishnagiri District, Tamil nadu-
635113, do hereby solemnly, affirm and declare on oath as follows
1) That I was student of M.B.B.S at P.E.S. Institute of Medical Science & College and
Hospital, Kuppam Andhara Pradesh state from 2013 to 2022.
2) That I have completed my compulsory rotatory internship training during 20.9.2021 to
27.12.2022 and I also extended internship from 20.9.2022 to 27.12.2022 for PES Institute of
Medical Science &College and Hospital, Kuppam, Andhra Pradesh.
3) That I could not get myself registered permanent with the Andhra Pradesh medical Council
due to the reason that I received my degree certificate belatedly.
4) That I have not done any unethical practice after completion of my internship training
However, if any complaint is made against me, I will be responsible for the same.
5) That I have not done any act amounting to disqualification for being registered under the
provision of section 15(4) of A.P. Medical Practitioner Registration Act 1968 (As amended
Act No.28/1986).
6) That the facts stated above are true and correct to the best of my knowledge and belief.
