This manual, is accepted or approved for USA Jet Airlines and is assigned to the individual
noted above for use as a guide to the safe and efficient performance of their duties.
It is the responsibility of each individual to whom a USA Jet Airlines manual is issued, to
maintain it in good condition, and to keep it current by complying with all revision insertions
promptly.
This manual shall be returned to USA Jet Airlines, in good condition, when recalled, or upon
termination of employment. In the event that an employee fails to surrender a manual, in good
condition, the employee will be charged $100.00 for each manual not so returned.

Contract agencies for this list will be categorized into two types: Essential Maintenance Providers and Non-
Essential Maintenance Providers (Repair/Overhaul Vendors (ROV). These vendors may be used to provide
maintenance, services, parts or equipment for USA Jet Airlines. The USA Jet Airlines Stores and
Maintenance Departments will monitor the use of vendors.
Letters of Agreement, Contracts or other legally acceptable arrangements may cover these transactions. A
file of Letters of Agreements or Contracts for Agencies/Providers for support to USA Jet Airlines will be
maintained by the Director of Quality Assurance; however, such contracts need not be maintained in the
Quality Assurance Area but must be readily available to the Director of Quality Assurance. In cases where
it is not possible to have either a Letter of Agreement or Contract with the vendor or organization, a USA
Jet Airlines Purchase Order will serve as a contract/agreement. Contracts for heavy maintenance checks
will specify if USA Jet Airlines or the contract agency will be filling any reports required under FAR 121.703.
NOTE: FAR 135.505(e) and 121.1005(e) require FAA Certified Repair Stations to be notified in writing of
USA Jet Airlines policies and operations specifications authorization permitting or prohibiting
carriage of hazardous materials, including COMAT. FAR 145.206(a) requires the repair station to
acknowledge receipt of this notice. The letter in GMM Volume 1, Chapter 5, Section 7, can be used
to notify repair stations of our policy.
All maintenance repairs or alterations accomplished on USA Jet Airlines aircraft, engines, or accessories
by contract agencies shall be accomplished in accordance with USA Jet Airlines, FAA Approved
Maintenance Program, Manufacturer's Manuals, Service Bulletins, or other data approved by the FAA.
In accordance with FAR 121.369(a), a “Standardized Maintenance Provider” listing of the FAA Certificated
Maintenance Facilities, Repair Stations, and Airlines that may be utilized by USA Jet Airlines, will be
maintained by Quality Assurance as a controlled manual under this cover. The Contract Agencies on file in
the Director of Quality Assurance office are those FAA Certified Repair Agencies most commonly utilized
by USA Jet Airlines, for the overhaul and repair of the company aircraft and equipment. This controlled
manual will be updated on a scheduled basis. Between updates, a current listing will be kept at USA Jet
Airlines in an electronic format which will be accessible by the Director of Maintenance, Maintenance
Control, the USA Jet Airlines Stores Department, and the EMI-FSDO. When the electronic version of the
provider list is revised the FAA Principal Maintenance Inspector at the EMI-FSDO will be notified within one
business day. The listing does not preclude the temporary use of any other FAA Certified Repair Station by
USA Jet Airlines, when circumstances warrant use. Listing of a vendor in the CASE Air Carrier Section
Register will qualify that maintenance provider for use to perform Non-Essential Maintenance.
USA Jet Airlines will perform surveillance of Essential/Non-Essential Maintenance Providers. This
surveillance with be conducted in accordance with the USA Jet Airlines Continuing Analysis and
Surveillance System (CASS) Program Manual. Audits of contract vendors will ensure they have an
organization adequate to perform the work and, if applicable, to perform RII Inspection. [FAR 121.365(b)]
MAINTENANCE PROVIDERS
Contract agencies that perform Essential Maintenance will be listed in the USA Jet Airlines Standardized
Maintenance Provider List as an Essential Maintenance Provider. No organization will be used to perform
Essential Maintenance (except as described in the last paragraph under this heading) unless such
organization has had an initial onsite audit performed and is then listed in the USA Jet Standardized
Maintenance Provider List. If USA Jet Airlines authorizes changes to the Essential Maintenance work that
an existing Essential Maintenance Provider will perform, and that change requires a change in work
category, then USA Jet Airlines shall accomplish another onsite audit that meets the requirements of
subparagraph e of Operations Specification D091.
A controlled copy of USA Jet Airlines General Maintenance Manual will be provided to those vendors who
perform Essential Maintenance for USA Jet Airlines on an ongoing basis. This manual will be controlled in
the same manner as all USA Jet Airlines copies of the GMM, and all revisions will be forwarded to the
agencies, along with a revision acknowledgement sheet. The revision acknowledgement sheet will ensure
all revisions have been received and inserted by the repair organization.
The Director of Quality Assurance, or his designee, will provide Required Inspection Item (RII) training to all
individuals identified to perform Required Inspections. This RII training will cover the same material and
information provided to USA Jet Airlines' RII inspectors. The Training Programs Coordinator will maintain a
list of all organization RII Inspectors.
USA Jet Airlines will ensure that all contractors used for Essential Maintenance are properly certificated by
the FAA to accomplish the services or repairs requested. The Director of Quality Assurance, or his delegate,
will appraise each Essential Maintenance provider prior to the first use and on a predetermined scheduled
basis thereafter. (Reference Continuing Analysis and Surveillance System (CASS) Program Manual).
USA Jet Airlines will perform surveillance of Essential Maintenance Providers (EMP).
This surveillance will be conducted in accordance with the USA Jet Airlines Continuing Analysis and
Surveillance System (CASS) Program Manual.
Because USA Jet Airlines is a Supplemental, “On Demand” Carrier, it may become necessary, from time to
time, to contract Agencies to accomplish Essential Maintenance. In such situations, the Director of Quality
Assurance may authorize an organization (Repair Stations or individuals) to perform such maintenance on
a one-time basis. The Director of Quality Assurance will verify the organization is certificated to perform the
required work, and the personnel assigned to accomplish the work have the background and experience
necessary to accomplish the required work. The Director of Quality Assurance, or his delegate, will discuss
the requirements with the agency's representative, and will obtain copies of the agencies certificates and
training records (as necessary), before making a final determination to allow the organization to accomplish
the required work. Reference USA Jet Airlines General Maintenance Manual GMM Volume 1, Chapter 4,
Section 3 for One-Time Required Inspection Item Authorization. A form M-57 must be completed prior to
use.
NOTE: M-57 Forms will be maintained in Maintenance Control for a period of not less than 90 calendar
days.
Contract agencies that perform Non-Essential Maintenance will be listed in the USA Jet Airlines
Standardized Maintenance Provider List as a Non-Essential Maintenance Provider. No organization will be
used to perform Non-Essential Maintenance (except as described in the last paragraph under the heading
of Essential Maintenance Providers) unless such organization is listed in the USA Jet Airlines Standardized
Maintenance Provider List.
Individual contractors that perform Non-Essential Maintenance shall be listed on the Standardized
Maintenance Provider List under the heading of “Maintenance Provider – Essential Maintenance – One
Time RII – Regularly Scheduled Maintenance.” This listing appears after the Non-Essential Maintenance
Providers near the end of the table listing. Individual contractors performing Non-Essential Maintenance
may be dropped from the provider list if not utilized again for a twelve (12) month period.
In accordance with FAR 121.369 (a) a “Standardized Maintenance Provider Listing” of the FAA-Certificated
Maintenance Facilities, Repair Stations, and Airlines which may be utilized by USA Jet Airlines will be
maintained by the Director of Quality Assurance as a controlled manual under this cover.

and equipment. This list will be revised as necessary. Between revisions, a current listing will be kept at USA
Jet Airlines in an electronic format which will be accessible by the Director of Maintenance, Maintenance
Control, the USA Jet Airlines Stores Department, and the EMI-FSDO. The listing does not preclude the
temporary use of any other FAA-Certified Repair Station by USA Jet Airlines, when circumstances warrant
use.
It shall be the responsibility of USA Jet Airlines to determine that all Agencies (Essential Maintenance/
Non-Essential Maintenance) used are Certified by the FAA to perform the services and repairs requested,
and be inspected by USA Jet Airlines once every 24 calendar months.
NOTE: If circumstances prevent a Non-Essential Maintenance vendor from being audited within the 24
calendar month period, it shall be permissible to exceed the interval by not more than 12 calendar
months, provided a risk-based assessment of the vendor to mitigate the risk of exceeding the 24
month standard is performed.
All maintenance, repairs, or alterations performed by any Contractor or outside Organization shall be
performed in accordance with the Manufacturers’ Manuals and/or Service Bulletins, approved Engineering,
and the USA Jet Airlines General Maintenance Manual.
The USA Jet Airlines Stores and Maintenance Departments will monitor the use of Contract Organizations
(Vendors (Stores Department) and Repair Stations (Maintenance Department)). Any maintenance
organization or individual not on the “Standardized Maintenance Provider List” may be utilized on an
occasional basis; however, a Form M-57 must be completed prior to use (refer to GMM Volume 1, Chapter
5, Section 7). Any Parts or maintenance organization meeting this criteria must be reported to the Director
of Quality Assurance.
NOTE: M-57 Forms will be maintained in Maintenance Control for a period of Not Less Than 90 calendar
days. Reference GMM Volume 1, Chapter 4, Section 4 for instructions on how to complete the Form
M-57
The Standardized Maintenance Provider List consists of a table and contains the following elements:

The table is split between Essential Maintenance Providers and Non-Essential Maintenance Providers. The
Essential Maintenance Providers will be listed in the beginning of the table and will have an (EMP) appear
in the Name of Agency block. Subsequent to the Essential Maintenance Providers the Non-Essential
Maintenance Providers will be listed. The table listing will be approved by USA Jet Airlines Quality Control
personnel and contain a date at the bottom of each page denoting the “Approved Date.” This table listing
does not require FAA-Approval. Also appearing at the bottom of each page will be a “Manual Posting Date”
which is the date on which the current listing is inserted into the hard copy manual. This will occur on or
about the first business date of each month.
The Standardized Maintenance Provider List will be updated on a scheduled basis. As required by FAR
121.368(h) the current updated list will be provided to the FAA and posted to this manual by the last day of
each month. If the last day of the month is not a business day, the updated list will be provided to the FAA
on the last business day for that month. Between updates, a current listing will be kept at USA Jet Airlines
in an electronic format which will be accessible by the Director of Maintenance, Maintenance Control, the
USA Jet Airlines Stores Department, and the EMI-FSDO. When the electronic version of the provider list is
revised the FAA Principal Maintenance Inspector at the EMI-FSDO will be notified within one business day.
Chapter 0 and Chapter 1 of this manual are accepted by the FAA and will be accounted for in the List of
Effective Pages. Chapter 2 is the actual Standardized Maintenance Provider Listing which is approved by
the USA Jet Airlines Quality Control Department which is responsible for auditing and approving vendors.
A USA Jet Airlines approval date for the listing will appear on the bottom of each page. The electronic
version of the table will reflect the same information.
