Instructions:
(1) Attach copy of (a) CNIC (b) Information / Allocation / Allotment Letter (c) Location Ballot Information Letter and
(d) 2 x passport size photos (Blue Background).
(2) Please pay issuance of PTM charges Rs 5,000 (Res) and Rs 10,000 (Comm) on DHA Challan (generated from DHA Multan Offices
only) & attach its DHA File copy. Alternatively, attach Bank Order in favor of “DHA Lahore – Multan Chapter”.
(3) Lien Marking Fee Rs 10,000 (Res) and Rs 20,000 (Comm) shall be payable at the time of lien marking by the
member.
