EVALUATION FORM

PURPOSE: This evaluation form is given to the HTE/FHE where the student
intern is deployed to help the faculty-in-charge (subject teacher)
in determining a fair grade to be given to the student trainee(s)
for his/her performance during his/her Internship. Further, this is
also intended to make the student trainee(s) be aware of his/her
strengths and weaknesses; and be able to know the areas in
need of improvement. Furthermore, this is designed to evaluate
the DSSC Student Internship Program (SIP). Your feedbacks will
be used as basis for improving the management of the Program
to maximize its benefits.

PROCEDURE: 1. This evaluation form should be accomplished by the student


intern(s)’ immediate supervisor to evaluate performance during
the conduct of the DSSC SIP.
2. The evaluator should rate the student intern(s) as objectively
as possible.
3. After filling in this evaluation form, the evaluator should
discuss the results of his/her evaluation with the student
intern(s).
