Medical Treatment Scheme, 2014' in respect of Shri Ambar Sundar Bose, son of Late Asit
Ranjan Bose and Late Maya Bose

of Late Asit Ranlan Bose, Ex-W.B.C.S. (Exe.) was
admitted at Rabindranath Tagore lnternational lnstitute of Cardiac Sciences (an empanelled HCO
bearing code-0411008 under WBHS, 2008), 124,MUKUNDAPUR, EM BYPASS,KOLKATA-7ooo99 on
L2.O5.2OZl for IPD treatment and passed away on 22.05.2021during hospitalization.
Maya Bose was enrolled under the West Bengal Health Scheme, 2008 vide l.D. No.
58112336lL/L. The Medical Cell of Finance Department has opined on 30.03.2023, "as there is no
living enrolled beneficiary of WBHS under late Maya Bose, digitalization of existing manual enrolment
card is not need ed."
Shri Ambar Sundar Bose has submitted the medical claim amounting to Rs. 3,74,959/-beyond
six months from the date of consuttation due to Pandemic situation. The delay in submission of bills
by Shri Bose has been condoned by the Medical Cell of Finance Department vide their U,O, No, E-
791-F (MED), dated-14.08.2023 and an amount of Rs. 3,51,589/-(Rupees Three lakh Fifty-One
thousand Five hundred and Eighty-Nine) only is found to be admissible as per instructions issued by
the Medical Cell of Finance Dep.rr ment

Shri Ambar Sundar Bose, son of Late Asii k.rnjan Bose and Late Maya Bose has also submitted
Death Certificate (issued oy Kolkata Munrcipal Corporation), Affidavit cum Legal heir, No Objection
Certificate and lndemnity Bond clarming the cost of medrcal treatment of Late Maya Bose.

ln the above mentioned flrcumstances, rhe Governor ls plea5ed to accord sanction to the
reimbursement of a sum of Rs. 3,51,589/-(Rupees Three lakh Fifty-One thousand Five hundred and
Eighty-Nine) only in favour of Shri Ambar Sundar Bose, son of Late Asit Ranjan Bose and Late Maya
Bose against the expenses incurred for lndoor treatment of her deceased Mother under the West
Bengal Health for Cashless Medical Treatment Scheme, 2014.
The above charge amounting to Rs. 3,51,589/-(Rupees Three lakh Fifty-One thousand Five
hundred and Eighty-Nine) only is debitable to the Head of Account "2o71-Pensions & other
Retirement Benefits-01-Civil-800-Other Expend itu re-00-002-Reimbursement of Medical Expenses
to Pensioners [FA] - 12' Medi€al Reimbursement under WBHS 2008" during the current financial
year.
The Deputy Secretary & DDO, Personnel & Adminisrrative Ref orms Department will act as the
D.D.o. in this regard.
This order rssues with the approval of the Additional Chief Sec retary of this Department.
