Immeasurable appreciation and deepest gratitude for the help and support are extended to the following

people who in one way or another have contributed in making this study possible.

Foremost, the researchers would like to express their sincere gratitude to their Clinical Instructor, fourth

year adviser, Ms. Shajanie Ganotice, RN, for the continuous support of our case study throughout these years of

nursing, for her patience, motivation, enthusiasm, and immense knowledge. Her guidance helped us in all the

time of research and writing of this case study. The researchers could not have imagined have a better advisor

and mentor for the case study.

Besides our advisor, the researchers would like to thank our best supportive, good adviser, insightful

comments, corrections to work duties, case studies, our one of our best unforgettable Clinical Instructor, Mrs.

Lourdes P. Abecia, RN. Her guidance helped the group to accomplish smoothly with this case study.

As a group, we would like to thank the rest of our previous Clinical Instructor, Mr. Roberto C. Osol,

RN, MAN; Mrs. Lesley Ann M. Pascual, RN, MAN; Mrs. Irene N. Lopez, RN, MAN; Ms. Janilyn Mae Saban,
