KNOW ALL MEN BY THESE PRESENTS:

That I, __________________________________________, of legal age, Filipino citizen and resident of


_________________________________________________. After having been duly sworn to in accordance with
law hereby depose and say;

That I’m the only legal heir of late ______________________________________________________,


who died intestate in the municipality of ____________________________________, but they left me
a parcel of land to their lawful heir.

That said parcel of land is morefully and particularly described and bounded as
follows to wit;

“A parcel of land located at San Antonio, Narvacan, Ilocos Sur, bounded in the North
by Lot No. 9101, bounded in the South by Lot No. 9097; bounded on the East by Lot
No. 9076; bounded on the West by Lot No. 9078; containing an area of 969 sq.
meters more or less and declared under ARP No. 14-0027-00315 and designated
under Lot No. 9077;” (description of the land often found in the Title Registration)

That the said parcel of land is free from all liens and encumbrances, not yet
register under Act 496 nor under the Old Spanish Mortgage Law, but this instrument
maybe registered under Act 3344 as amended.

IN WITNESS WHEREOF, I have hereunto set my hand and signature this day
