LOTO Procedure
Purpose:
This Standard Operating Procedure (SOP) outlines the Lockout-Tag Out (LOTO) procedure to ensure the
safety of personnel working on Ley Choon Constructions and Engineering Pte Ltd’s construction sites.
LOTO is essential for preventing accidental energy releases during maintenance, servicing, or repair of
equipment, machinery, or systems. Adhering to this procedure will minimize the risk of accidents and
injuries.
Scope:
This SOP applies to all employees and contractors working on construction sites under the jurisdiction of
Ley Choon Constructions and Engineering Pte Ltd.
Responsibilities:
Site Supervisor Responsible for overseeing and approving the
LOTO procedure for specific tasks.
Authorized Employee Trained and authorized to implement LOTO
procedures.
Procedure:
1. Preparation:
a. Identify all equipment, machinery, and systems that require LOTO.
b. Assemble the necessary tools and equipment, including padlocks, tags, hasps, and lockout devices.
c. Obtain authorization from the site supervisor for LOTO.
3. Shutdown:
a. Isolate the equipment or system from its energy source.
b. Turn off all switches, valves, and control panels.
c. Follow the manufacturer's guidelines for equipment shutdown if available.
4. Lockout:
a. Attach the LOTO device to the energy-isolating point, ensuring it prevents re-energization.
b. Each authorized employee must apply their personal padlock to the LOTO device.
c. Ensure that no one else can remove the padlocks except the authorized employees.
5. Tag out:
a. Attach a LOTO tag to the LOTO device, indicating the reason for the lockout and the responsible
personnel's name and contact information.
b. Include a warning against unauthorized removal.
6. Verification:
a. Verify that the equipment is safely de-energized by attempting to start or energize it.
b. Ensure that all employees are clear of the equipment.
c. Confirm that all LOTO devices, locks, and tags are securely in place.
7. Maintenance/Repair Work:
a. Conduct maintenance, repair, or servicing work while the equipment is locked and tagged out.
b. Use caution and follow safety procedures throughout the task.
8. Completion:
a. Once the work is completed, remove tools, materials, and personnel from the work area.
b. Verify that all employees and equipment are clear of the work area.
9. Removal of LOTO:
a. Unlock and remove the LOTO devices, padlocks, and tags.
b. Energize the equipment or system as needed for operations.
