- Monitor the Vital Signs
- Observe for skin irritation.
- Encourage the patient nutrition and hydration.
- Advise the patient to keep the skin clean and dry.

Dependent
- Administer diphenhydramine q8, PRN as per
doctor’s order.
- Administer cetirizine 10mg tab BID as per doctor’s
order.

RESPONSE
- Patient report sign of relief from itching caused by
the rashes.
- Patient reports absence of any rash or irritation.
- Patient was able to follow the health teaching with
ease
